This form applies only to returns to be filed in 2005 for tax year 2004.

(Oct. 15, 2004 is the last day the IRS will accept e-file returns for tax year 2003.)

Before you can use the unlimited e-file features built into Tax Preparer Standard and Premium Levels, you must annually register and tell us how you will pay for the transmission fees and how you want to be paid the commissions you earn on bank products. The transmission software supplied with Tax Preparer cannot be activated until we receive the information on this form. Once you have completed this form, you can submit, print, or cancel the order with one of the buttons at the bottom of the form.

Transmission Fee Arrangements:

 The standard charge per return is $9.95 federal, $5.95 state, for the e-file Bronze that is built into Tax Preparer Standard and Premium Level software. Lesser charges apply if you purchase an e-file special to Upgrade to Silver ($5.95 federal, $3.95 state), Upgrade to Gold ($2 federal, $1.50 state), or Upgrade to Platinum (NO per-return fee). Arrangements for paying these fees must be made now through your completion of the following entries. If you choose to be billed, you must pay the periodic bills as invoices are received or your participation in the e-file program may be revoked.

Bank Products Sign-up

Payment of $49 sign-up fee:

 There is a $49 annual charge to participate in this bank products program. Payment of this fee qualifies you to offer RAL and QIK Funds to your clients. RAL fees range from $33 to $89 depending on the size of the refund borrowed, which are automatically deducted from the refund received from the IRS. QIK Fund fee is $24.95 irrespective of the amount of refund, which is also automatically deducted from the refund received from the IRS. RALs are automatically rolled over into QIK Funds if the client does not qualify for the requested loan.

I Decline

If you decline you will not be able to offer Refund Anticipation Loans (RALs) or QIK Funds through your e-file transmission software, and you will receive no commissions.

 You will receive commissions for any RAL or QIK Funds products you use at the rate of $4 per RAL return and $3 per QIK Fund return. You can choose to have the commissions distributed to you weekly by direct deposit into your account or twice a month by cashier's check to you.

Cashier's Check

If you choose to receive your commission by check, a cashier's check for commissions will be distributed to you on the 1st and 15th of each month.
